Sometimes yes, sometimes no. A lot of BS rumors float around and muddy the waters, so people might be aware of it as a rumor that exists, but they won't know if it's true or not.
Either that or Jungkook, V, and Jimin are each simultaneously dating every member of BP, Twice, and RV. Keep an eye out for the Dispatch report of the century

Believe it or not, for some idols, the idea of someone is in love with them is actually absurd. It makes them laugh. LOL.

I think international fans REALLY overshoot the idea of Korean fans knowing about stuff. For example no one knew about Kang Daniel/Jihyo even though Daniel has super dedicated sasaengs.
In some cases, yes, they get lucky and get information, especially if the person these idols are dating are super obvious about it, like Chen's wife was.

I think that people who live or work in the same areas that idols live or work probably see them out and about and pick up on it. There are a lot of fans who will just tweet that they saw this idol buying coffee and they looked handsome/pretty/tired, but don't really try to spill details about them.
Then their are their saesangs who definitely know. Some of them keep it secret because they're worried about how it will effect their chosen idol and some use it as leverage against them.
