It's always the opposite for me. Always.
The sex scenes in particular are often weird. Like there's some unwritten rule that sex has to occur at some point even if it doesn't add anything to the plot and its totally random.

There's no general rule, it depends on the movie. But yes, some movies could use a few more shock elements when their intent is to be gritty and disturbing. For example, some movies build tension up until an expected explosion of violence, yet when the outburst happens it's very lame and defuses everything the movie tried to build. In which case, it's bad.
Violence, sex, profanity... everything may have a reason to be in a movie; it just has to make sense in the context of the movie, according to what the movie is trying to achieve.
