I used to have the same problem, but read somewhere that developing a night routine before going to bed lets your body adopt a specific sleep cycle.
It can include reading a page or two of a book for example.

Melatonin (3-5 mg) or Benedryl (50 mg) 30 minutes - 1 hour before bed
Do not use use your bed for anything except sleeping
Do not "try" to go to sleep. Read a book or something that doesn't involve a screen.
Avoid alcohol (if you're of age)
Avoid caffeine products or stimulants, preferably 6 hours before your usually bedtime
Dark = Better
Warm baths may help
Consistent sleeping AND waking schedule
Don't drink or eat anything before bed
Also if you have clinical depression, it should be treated with antidepressants and be resolved since insomnia commonly coincides with depression

Perhaps seek a professional? I don't know the whole story and just going by your brief description and I'm sure the people here are well-meaning but if its bad as you say and there's the depressive component to it, maybe a health care professional would be more qualified to give you the help you need.

i saw that exercise helps depression, try excising throughout the day that way you'll be tired and your mind will be focused on something else

drink hot milk
i used to do this when i had sleep problems, hot milk and a little sugar really helped me

It sounds like stress might be a big factor. Only a doc can tell how bad it really is.
What has helped me in the past is aromatherapy. Lavender essential oil or plug-in helps to relax your mind and body. I have also combined two kinds of relaxing-type teas like Sleepytime Tea and Chamomile Lavender or Peppermint tea and drink it a couple hour before bedtime.
