Theme: (You can ask for a theme or a pattern as example hearts/snow/universe,... you are only allowed to pick 1 theme or pattern)

Emoji: (You can only say if you want emoji or no emoji in your signature)

If you don't send the filled in form, I will have to ignore you

All signatures are going to be 300pixel tall and 600 pixel wide
