I remember this was the same "before-after" photo that MNET showed, so I think they wanted to make it look drastic. I follow him since a long time ago and he still looks the same, even live, even with PS. You can see that in this photo and in the other 2-3 photos, which were taken at a specialized studio, his face looks a little bit different. Those are the ones photoshopped, you can see clearly that his face looks slimmer, but in reality, it's not that slim. Also, makeup makes it looks more drastic.
He got PS, but it's not as extreme as these pictures portray.
